No, you only need an Umweltplakette sticker if you enter the German Umweltzones located various cities around the country. This is regardless of whether the vehicle is German or non-German registered. My own personal car, a UK-registered SAAB, has an Umweltplakette because I was staying at a friend's house within the Bremen Umweltzone some years ago.
German cars are not required to have the sticker if they're used outside of these Umweltzones. I have spotted many legitimately-registered German vehicles without Umweltplakettes, even in Germany!

That's right! In addition, there's several categories of cars which don't need a sticker. Historic vehicles are exempt from a certain age (I think it's 30, but I'm not sure) and there's a few other exceptions as well. Plus, if you don't ever travel to an Umweltzone, like you said, you don't need a sticker.
