how do the county codes for Texas work anyway?

Basically, they're just distributed consecutively and boxed up and shipped to certain counties... Because they are in order, you can "abuse" the letters and use them for location. But a Prefix usually is shared with other smaller counties, like BKC for example, 0400 - 2399 is Val Verde County (Del Rio), after 2399 probably went to another small county, but the rest of it went to Harris County (Houston)...
